Tissue doppler–derived diastolic function parameters and their association with TyG index and inflammatory ratios in pregnant women with dyspnea

Abstract

Distinguishing high-risk women presenting with dyspnea during pregnancy remains clinically challenging. This study aimed to investigate the association between TDI-derived diastolic function parameters and inflammatory–metabolic indices, including the triglyceride–glucose (TyG) index, hemoglobin-to-RDW ratio (HRR), RDW-to-albumin ratio (RAR), and uric acid-to-albumin ratio (UAR), in pregnant women presenting with dyspnea. These indices reflect inflammatory and metabolic alterations that may influence cardiovascular adaptation during pregnancy. A total of 255 pregnant women were evaluated, including 130 with dyspnea and 125 controls. Demographic, laboratory, and echocardiographic parameters were recorded. TyG index, HRR, RAR, and UAR values were calculated and compared between groups. Receiver operating characteristic (ROC) analyses were performed to assess classification performance. Compared with the control group, pregnant women with dyspnea had significantly higher TyG index and UAR values, whereas HRR and RAR did not differ significantly. TDI-derived echocardiographic assessment revealed larger right atrial and right ventricular diameters, higher systolic pulmonary artery pressures, reduced septal and lateral E′ velocities, and increased E/e′ ratios in the dyspneic group. ROC analysis demonstrated that BNP, D-dimer, and UAR showed an acceptable discriminative performance for dyspnea (AUC = 0.747, 0.730, and 0.713, respectively), whereas the TyG index showed modest discriminative ability (AUC = 0.625). These findings suggest that TyG index and inflammatory ratios were associated with subtle statistical differences in selected tissue Doppler–derived parameters and may warrant further investigation in conjunction with clinical and echocardiographic assessment.
